REPORTING DEATH OF A TEXAS NARFE MEMBER |
There are now two methods to contact OPM about
the death of a federal employee or retiree other than by mail.
One is the main 1-888-767-6738 toll free number, available from
7:30am EST to 7:45pm EST. The other is by email at retire@opm.gov.
Members can complete a report of death on-line and email it by going
to OPM's Retirement website at: https://apps.opm.gov/retire/death/death.cfm.

You should report the death of a NARFE Chapter
member to our Texas Federation of Chapters (TFC) Secretary and to
Headquarters NARFE. The TFC Secretary will adjust TFC Records
and notify the Editor of the Texas Federal Retiree (TFR) of the
death so the death will appear in the next issue of the TFR.
Headquarters NARFE will use the information to adjust records at
that level. There are two acceptable methods of reporting
death. The Chapter Secretary or other official designated by
the Chapter President should make the death notification.
Whichever method is used, ask for confirmation of termination of
membership dues with holding for the deceased. The FIRST
method which is probably the easiest involves the use of
email. Simply send an email to memberrecords@narfe.org
at Headquarters NARFE with a copy to Fran Hiner-Pantoja our
TFC Secretary at franhiner@sbcglobal.net.
You may send a separate email to Headquarters and to Fran
Hiner-Pantoja. However, be sure to include the member's name,
chapter number, date of death, their NARFE membership number, and
their surviving spouse and address if known. The SECOND method
involves use of the Headquarters NARFE Form Number F-9, dated
08/02. The F-9 is a prepaid postcard addressed to Membership Department, 606 N. Washington St.,
Alexandria, VA 22314-9704. An printable copy of
the F-9 is available by clicking here (It
should be printed double sided). For information purposes, a
copy of the backside of a F-9 is presented below. If you mail
the F-9 in a separate envelope, be sure it is addressed to the above
Alexandria, Virginia address. You can advise the TFC Secretary by
mailing a copy of the F-9, sending an email to franhiner@sbcglobal.net
, or by a separate letter to: Fran Hiner-Pantoja TFC Secretary 9211 Standing Creek Ln. San Antonio, TX 78230-4040 Her telephone is: (210)
380-8743 The TFC Secretary needs as a minimum the
member's name, chapter number, and date of death. Do not send
the report of the member's death to the Editor of the Texas Federal
Retiree, since the deaths are reported to the Editor by the TFC
Secretary. |
